changes to your personal circumstances either by contacting the office
directly or by writing to us.

We have legal obligations under the Data Protection Act to ensure that all
information held and processed about you complies with principles of the Act.

The Act requires all personal information to be treated in the strictest confidence and to be used only for purposes of which you are aware.

The type of information we will gather about you will include your disability and how it affects you. We will also require details of how to contact you and /or a named other person in the event of an emergency.

In addition we will ask you to provide details of any medication you are taking and to update us if and when any changes are made to your medication.
